Chronic Illness

Students with chronic illness conditions (defined by ARS 140-901 as “a pupil who is capable of profiting from academic instruction but is unable attend school due to illness, disease, accident or other health conditions, who has been examined by a competent medical doctor and who is certified by that doctor as being unable to attend regular classes for a period of not less than three school months or a pupil who is unable to attend regular classes for intermittent periods of time totaling three school months during a school year”) should obtain chronic illness request forms from the nurse’s office at the beginning of the each school year. Examples of chronic illnesses include severe asthma, uncontrolled diabetes, mononucleosis, cardiac problems, debilitating migraines, etc.
 
Dispensing of Medications

All medications must be kept in the nurse’s office. Students are not allowed to carry medications while on campus. A signed medication permission must be on file in the health office for all prescribed medications and over-the-counter products to be dispensed. Students must bring in their own bottles of pain relievers, cold medication, allergy medication, antacids, and eye drops (in the original container), as Sweetwater does not provide these types of medications. Asthmatic students may carry their own inhalers with a written doctor’s prescription on file in the office. Any student with undocumented medications on their person will be subject to disciplinary action.

Immunizations


Medical records showing dates of immunizations must be completed and on file for each student. An emergency card must also be completed and signed by both the parent/guardian and student. This card will be kept on file in the front office.

Ill Students

Students who are ill need to be seen by Nurse Charlotte prior to being released from school. A parent or emergency contact person will be contacted and asked to please come and pick them up. For the well-being and safety of our students we do not release ill students to go home alone.
